RN Intake Coordinator

Schedule:

Standard operating hours are 8:00 am – 5:00 pm; Monday-Friday. Specific work hours will be established through discussions with your supervisor.

Job Summary:

RN Intake Coordinator is responsible for driving enrollment and referring patients to promote healthy growth of the program and to enhance market awareness of the PACE program to potential referral sources and to generate referrals of potential participants within the site area.

Duties/Responsibilities:

  • Meet with prospective enrollees and utilize Nursing assessment skills to determine eligibility of potential participants for the PACE program.
  • Responsible for all aspects of the Home Visit process: explaining PACE, completing all initial intake assessments, signing enrollment paperwork, etc.
  • Communicates regularly with Participant Advocate to ensure Interdisciplinary Team (IDT) has all needed information on the potential participant.
  • Coordinate services with State Health and Human Services Personnel.
  • Participates in team and training related meetings and activities.
  • Coordination with IDT relative to initial assessments
  • Participates in marketing activities in conjunction with the Marketing Manager, as needed and available.
  • Implementation of the “steps in the intake” process to throughput potential participants into LIFE enrollment
  • Collects, completes and keeps confidential appropriate documentation related to participant assessments as required.
  • Maintains required statistical data.
  • Participate in and support Quality Improvement initiatives.
  • Other duties as assigned.

Required Skills/Abilities:

  • Must be able to deal tactfully and patiently with personnel, patients, visitors, and the public.
  • Ability to organize work and follow through on assigned tasks.
  • Excellent verbal and written communication skills and can follow written and/or oral directions.
  • Must possess a valid driver’s license and reliable transportation.
  • Must have the cognitive ability to process and perform tasks for Participant safety.
  • Must work effectively in a team environment and possess excellent communication, interpersonal and conflict-resolution skills. Able to work with adult geriatric patients and their family members.
  • CPR and First Aid Certification.

Education and Experience:

  • Bachelor’s degree in nursing
  • Possess a current unrestricted KY State nursing license
  • Two years of nursing or case management experience
  • One year minimum of experience working with the frail and elderly population or willing to participate in extensive training as needed.
  • Knowledge of Medicare/Medicaid regulations a plus.

Requirements:

  • Requires valid State driver’s license with a clean driving record.
  • Successful completion of background check, physical, drug screening, TB Screening

Physical Requirements:

  • While performing the duties of this job, the employee is frequently required to stand; walk; lift heavy objects; sit; use hands to finger, handle or feel objects, tools, or controls; talk, see, hear and smell.
  • Requires full range of body motion, manual and finger dexterity, and eye-hand coordination; requires the ability to use department equipment to communicate effectively; The employee is occasionally required to reach and stretch with hands and arms; climb or balance; and stoop, kneel, crouch, or crawl.
  • Must have the ability to work under moderate pressure to meet scheduled appointments or deadlines while dealing with frail, disabled and/or confused participants/clients.
