Contract Physical Therapist (PT) – Mountain View PACE

Contract Length: 6–8 weeks

Position Summary:

Mountain View PACE is seeking a Contract Physical Therapist (PT) for a 6–8-week assignment supporting our interdisciplinary care team. The PT will assess, plan, and provide restorative therapy to promote participants’ functional mobility, independence, and quality of life. This role includes evaluating participants, developing individualized plans of care, and coordinating with the interdisciplinary team (IDT) to meet goals and ensure safe, effective care.

Key Responsibilities:

  • Conduct initial and ongoing physical therapy assessments and evaluations.

  • Develop individualized treatment plans and therapy goals based on participant needs.

  • Provide therapeutic exercise, gait training, pain management, and restorative care.

  • Recommend durable medical equipment, orthotics, and prosthetics as appropriate.

  • Document all care and interventions accurately in participant medical records.

  • Educate participants, caregivers, and staff on therapeutic techniques and safety.

  • Provides visits and/or treatment in various settings, including day centers, Participants’ homes, skilled nursing facilities, or hospitals.

  • Participate in interdisciplinary care planning and 24-hour care coordination.

  • Support quality improvement and wellness initiatives.

Qualifications:

  • Current Physical Therapist (PT) license in Kentucky (required).

  • Minimum one year of experience working with the elderly or frail populations (preferred).

  • Strong clinical competency in geriatric care and mobility rehabilitation.

  • Excellent communication and documentation skills.

  • Reliable transportation and a clean driving record.

  • CPR and First Aid certification

Requirements:

  • Successful completion of background check, physical, drug screening, TB Screening.

Physical Requirements:

  • Ability to sit, stand, bend, lift, and move intermittently throughout the day.

  • Must be able to safely lift and support participants and equipment.
